Pthophysiology
Anemia leads to decreased level of blood volume in intravascular region and then leads to hypovolemia and hypoxia, this hypovolemia leads to hypotension, this hypotension is detected by stretch receptor and these stretch receptor trasmit impulses to the affrerent nerves of the vagus and glossopharyngeal nerve to the medulla oblongata, cerebral cortex and pituitary gland, then sympathetic impulses increased leads to norepinephrine from the sympathetic nerve ending and release of norepinephrine and epinephrine from the adrenal medulla, then sympathetic connection leads to increased ADH from the pituitary gland
ADH leads to reabsorb the free water in DCT and in response to decreased perfusion, juxtaglomerular cells of afferent arterioles release renin in renal circulation, then renin leads to release of angiotensinogen I, then it is coverted by ACE and coverted into angiotensinogen II.
Angiotensin II has a potent pressor effect on arteriolar smooth muscle.
Angiotensin II also stimulates the zona glomerulosa of the adrenal cortex to produce aldosterone
Aldosterone increases sodium reabsorption from the proximal tubules of the kidney, thus increasing intravascular volume.
